This is the request form for the Custom Print service. It includes a design created by me of your choice and a physical print delivered to you all for $75! You can choose from the following print sizes:

• 8.5" x 11"
• 11" x 14"
• 11" x 17"
• 12" x 18"

There are 2 revisions given in this project. In the event that an additional revision is requested, it will be an additional $35 and due before the next revision can begin.

The project is estimated to take 2-3 weeks.
